OC Chorus Sweatshirt & Shirt Order
Hooded Sweatshirts are $20, Long-sleeved shirts are $15 and Short-sleeved Shirts are $10. All money and orders are due by August 31

Payments may be made by cash or check made out to OCHS. We will also accept payments through the OCHS Payment Portal. Once you have submitted this form, please go straight to the portal to make a payment or send in your payment by Aug. 31.  

HOW TO ACCESS PAYMENT PORTAL: Please go to the Parent Tab on the OCHS homepage and choose Online Payment System (non-athletic).  Click Pay With Credit Card. From there choose Chorus.  There will be the option to choose Hoodie or -TShirt.    The link will also be provided on the confirmation screen.  Please don't hesitate to contact me if you have any questions.
