Dear Students and Parents-

Please complete this form together to pick out your classes for next year. If you have questions about certain courses, don't worry, we will meet individually to discuss them together.

*Also keep in mind that all 12th Graders are required to be enrolled in at least 5 classes in the fall (if they are on track for graduation). All students are required to enroll in at least 4 LVIS classes.
