This blanket consent form is for the Parent / Caregiver who is taking their child to an event/trip that is not a College Sport Sanctioned Event (which is not covered under College Sport (e.g. Code of Conduct, Health & Safety and Bylaws).

Parent / Caregiver are completely responsible for their child and themselves, Orewa College is not responsible for any of the below agreements.

This permission form is for the those parents who are taking their child and / or other students to an event that is outside of school hours on a weekend - "A parent lead trip".

I acknowledge the need for my child as well as myself to behave responsibly while participating at the event, representing Orewa College, according to the Orewa College Manaaki values

I acknowledge that this event is not a college sport santioned event, therefore is is not covered by the College Sport Bylaws (e.g. Code of Conduct/Health and Safety).

I understand that the school does not accept responsibility for (anything).

I recognise that participation in such activities is voluntary and not mandatory. My child and I both understand that they may withdraw from this event if they feel at risk. This must be done with the consultation of the event organiser

I understand that the school does not accept responsibility for loss or damage to personal property (either my childs property or damage to other property caused by my child) and that it is my responsibility to check my own insurance.
