I understand that in order for my child to participate in this year’s school play, he/she must adhere to the following:

Follow all school rules and behave in an appropriate and respectful manner as stated in their Brea Olinda Elementary School Parent-Student Handbook.
Maintain passing grades and complete all classroom responsibilities; participation is at teacher’s discretion.
Attend all scheduled rehearsals unless excused.

Costumes fees are the responsibility of the parent. Detailed costume information will be available later.
Volunteering is mandatory. Every family must signup for a volunteer committee and fulfill their commitment. Please sign up at the parent meeting or during auditions.

To ensure the safety of the cast members and the professionalism of the program, Stagelight Productions and Fanning PTA reserve the right to dismiss cast members who exhibit unacceptable behavior.

I understand that participation in “Beauty and the Beast.”, including required transportation (“the Activities”), is not without risk. With full knowledge of such risks, and in consideration of being permitted to participate in the Activities, by filling this form out I hereby fully assume all risks, known and unknown, of illness and injuries, even if caused by the action, inaction, or negligence of Fanning PTA, Stagelight, BOHS or any City official, employee or volunteer (“Released Parties”), to the fullest extent allowed by law. I further agree to indemnify, defend and hold harmless the Released Parties, with respect to any and all claims and liabilities for bodily injury, illness or death, in any way arising out of my participation and my child’s participation in the Activities, to the fullest extent allowed by law. I hereby authorize Released Parties to render emergency medical care if deemed necessary, and I further agree to pay all costs thereof. I further authorize Fanning PTA and Stagelight to use any photograph, video or other image taken of me or my child during the Activities, for any purpose, and without compensation.

Volunteering is mandatory. We cannot make this happen without your help!! Every family must sign up (Minimum of 5 hours per family) for a volunteer committee(s). Under the direction of the chairperson, volunteers will complete tasks associated with their committee. At least one parent from each family must volunteer to work on one or more committees, giving a minimum of FIVE hours of time throughout the production. Pre-sale ticket orders will only be processed for families that have volunteered for a committee. *

Cast Party- 12/21 @ BOHS, 3:30pm-6:30pm- Plan and organize the cast party. Volunteers will help set up before the party, work during, and help clean up after the party.

Concessions-Coordinate donations for concessions; set up and organize concession stands during performances. Volunteers will help with the above as directed; sell items 1 hour before performances and during intermission. Each family will be asked to donate items to be sold at concessions. Item choices will be posted at a later date.

Costumes-Help pick-up costumes at warehouse. organize, distribute, track, and collect costumes and return to warehouse; assist lead cast members with costume changes; under direction of producer - utilize make-up to create characters’ looks. Volunteers will help keep costumes in order and will assist children with costume changes; may also help with hair and/or make-up.

MIB (Men/Moms in Black)- Under the direction of the producer, run scene changes and move set pieces during tech week and performances

Load in/ Strike-Set); Load In (12/14, time TBD): Volunteers will meet at Stagelight warehouse and then proceed to BOHS to assemble set under the direction of Stagelight staff. Strike set (12/22 after last show) and ensure all is returned to storage at warehouses.

Supervision-Provide supervision of children during audition week, tech week rehearsals, and performances. Help students with costume changes backstage when necessary and keep a safe, well-organized, and fun environment backstage during all p.

Gift Sales- Purchase candy and gifts (with reimbursement), set up and organize sales table during performances. Volunteers will help with the above as directed; sell items 1 hour before the show and during intermission.
