CAS Service Provider Request Form 
Who Should Use this Request Form? 
This form is for current students registered with Communication Access Services (CAS) to request a service provider (i.e., interpreter, transcriber) for a single, UMD-related event.   

Examples of UMD-related events include but are not limited to:
  • Instructor meetings
  • Advising appointments 
  • Group project/meeting
  • UMD event required for class
Services are Subject to Availability
If a request is made with less than two weeks’ notice, there may not be enough time to schedule a provider given the high demand for services in the area. However, if an immediate need arises, please contact the DHHS office as soon as possible.

NOTE: Do not use this form to request a provider for ongoing services (i.e., semester classes). Instead, please email ads-cas@umd.edu directly to discuss those needs.
