This form allows my child/ren to charge only a regular hot lunch to the family account. All other items such as milk, ala carte or second servings will not be allowed. These items can be purchased with cash.

*This request will be honored whenever possible but we cannot be accountable if there are computer problems, substitutes, etc. of which we have no control. Please notify your child/ren of this restriction. This notice will be effective from the date it is received in the main Food Service Office for the school year 2023-2024.
