We are so excited for you to join us for AMHCA's 2021 Virtual Conference! Because you originally registered for our event when it was still onsite, you may now have a balance because our costs have now decreased. Below you can let us know below how you'd like your balance to be directed. Once you have filled out your form, you will receive an email from AMHCA Staff confirming your choice. Please allow 5-7 business days to receive confirmation from AMHCA Staff - we are working around the clock to ensure the success of this event and will reach out to you as soon as we possibly can. Please do not reach out before 7 business days.

We ask that you fill out this form no later than April 30, 2021. If no response is received by May 1, 2021, we will proceed with Option 2, and your balance will be added as a credit on your AMHCA account with an additional $25 bonus credit.
