Registration Waiver, Medical Release, and Drop Off/Pick Up Responsibility Form

I, the undersigned, hereby acknowledge and agree to the following terms and conditions set forth by Seattle Persian School:

 Liability Waiver:

I understand that Seattle Persian School staff take every precaution to ensure the safety of all children while at Seattle Persian School and on field trips, and that my child is expected to abide by all the rules and regulations of the school at all times. My child has my permission to participate in the programs and projects provided by Seattle Persian School. I fully understand that some of Seattle Persian School' programs may include the use of tools, equipment, and materials that may present a danger to my child. I acknowledge that prior to being given permission to participate in these activities, my child will receive studio/classroom/shop/kitchen safety information and instruction in the appropriate rules and practices which apply to the materials, tolls, and equipment used as part of these programs. My child agrees to observe all safety rules established by Seattle Persian School' faculty and staff. They understand that violations of these rules may result in the withdrawal of privileges for certain tools, materials, and equipment, and/or suspension from participation in Seattle Persian School' projects, or even expulsion from Seattle Persian School itself. I agree that I will not hold Seattle Persian School, their personnel, or the chaperones responsible for any accidental harm which might befall my child while at Seattle Persian Schoolor on school trips. I acknowledge that I am signing the agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

 Registration and Attendance:

 ·      I understand that registration is complete only upon submission of the required registration form and payment.

 ·      I acknowledge that Seattle Persian School reserves the right to cancel any class if it does not reach the minimum number of registrants. In such cases, a full refund will be issued.

 ·      If a class is canceled for any reason, Seattle Persian School will make every effort to provide makeup options, which may include substituting teachers or offering online classes. If makeup classes are not feasible, the tuition for the canceled session will be refunded to the customer.

 ·      I understand that refunds will not be issued for cancellations made with less than 30 days' notice.

 Medical Release:

 I grant permission for any necessary medical attention to be administered to my child/student, in the event of an accident or injury, and I will assume all costs associated with any medical care.

 Drop Off/Pick Up Responsibility:

 ·      I understand that it is my responsibility to drop off my child/student at the designated location and time specified by Seattle Persian School, and to ensure that they are picked up promptly after each class or activity.

 ·      I will notify the school in advance if someone other than myself will be picking up my child/student.

 Media Release: I grant Seattle Persian School permission to use photographs or videos of my child/student for the purpose of promoting the school on its website or other social media platforms. I understand that these images will be used in a culturally sensitive manner and will not reveal personal identifying information.

 By signing below, I confirm that I've read, understood, and agree to adhere to the terms during my child/student's enrollment at Seattle Persian School.
