PARENT’S APPROVAL, STUDENT, FAMILY, AND PARTICIPANT WAIVER
All new and returning Almond families, must sign the PTA Student and Participant/Volunteer Waiver once every school year!
The California State PTA requires each family to sign the waiver in order for students and their families to participate in PTA-sponsored events, such as the Ice Cream Social, Walkathon, Family Science Night, book fair, Field Day, etc.
Only one waiver is required per family per school year. Please indicate the names of your students attending Almond Elementary, as well as any family members who might participate in PTA events including siblings, cousins, parents, grandparents, guardians, babysitters, etc.
