Payment is handled via PayPal. This is the email address associated with your PayPal account that we should send your invoice to.

This information will be provided to the hotel as we're working on the menu for Meet & Greet, Friday lunch, and the buffet at the Saturday dance. All meals are subject to change prior to the release of the 2024 schedule. 

Fannish name, wallet name, user name, whatever name you'd like to have on the front of your badge. Please keep this under 20 characters.

If you have an alternate name (a different username that might be recognizable, or a nickname) that you'd like us to print on the back of the badge, put that here.

These will not be included on the pre-printed badge, but you're welcome to write them on! We'll also have some pronoun buttons available at the con if you want to display them more prominently.

Please provide your Discord username. We will be setting up channels on our public Discord, accessible to attendees only, to use on the ground during the convention for planning room parties, group activities, etc. If you join our Discord, we will give you a role when the channels become available.

Any person who refers a new CONfab attendee will get 10 extra tickets for the CONfab charity raffle!

I acknowledge that I have read the CONfabulation COVID-19 Policies (the "Policies"), and I agree to abide by the Policies when attending the Convention. I understand and acknowledge that any in-person gathering carries an inherent risk of transmission of COVID-19 and other communicable diseases. I further understand and acknowledge that CONfabulation has no control over the actions of non-attendees in the hotel outside of con spaces, who are not bound by CONfabulation's policies and who may take different or lesser safety precautions. CONfabulation cannot and does not guarantee that attendees will not be exposed to or infected with COVID-19 or any other infectious disease. I acknowledge and agree that I voluntarily assume these risks by choosing to attend CONfab Chicago 2024.

To the fullest extent permitted under applicable law, I, on behalf of myself, my family, successors, heirs, executors, agents, assigns, and representatives, expressly waive, release, agree to hold harmless, and forever discharge Winconferences Next Gen, LLC d/b/a CONfabulation and its officers, representatives, agents, assigns, staff, volunteers, and legal successors from and against any and all claims, suits, demands, actions, causes of action, losses, damages, expenses, or liability of any kind whatsoever resulting from or arising out of any illness, injury, or death associated with infection or transmission of COVID-19 or any other communicable disease in connection with my attendance and participation in CONfab Chicago 2024.
