If you are reading this form, we trust that you have read through the Mission Initiative brochure and had a conversation with your Council leader connector to discuss the mission outreach being done and how you can connect with these opportunities.

Please consider the following options of how you can support Concordia Church this year and mark those that apply.

Please note: this form is to be completed for one individual at a time. If there is more than one member of your household, each person should fill out a separate form.
