Beginning September 7, 2021, parent(s)/guardian(s) will have the option to pick-up meals for students under quarantine at 4 different Elementary locations 1) East Bend Elementary, 2) Yadkinville Elementary, 3) West Yadkin Elementary, 4) Jonesville Elementary.  

In order to reserve meals for pick up during your child's quarantine, parent(s)/guardian(s) must complete this form by noon the day prior to meal pick-up. Meals will include one breakfast and one lunch for that day's service.
You may order meals up to 5 days in advance. By completing this form, you are agreeing to pick-up the meal on the selected day at the selected pick-up site between 9:45 a.m. and 10:15 a.m.

If you must cancel your order, please contact the school office at your selected meal pick-up site.
