You consent to be photographed, filmed and/or otherwise recorded. You consent to such photography, filming and/or recording and to any use, in any and all media throughout the universe in perpetuity, of your appearance, voice and name for any purpose whatsoever in connection with the production presently entitled: Swamplight Theatre, Kay Butler Performing Arts Project (“ST KBPAP”). ST KBPAP is committed to building a community in which all persons can work together in an atmosphere free of harassment, exploitation, or intimidation. Failure to demonstrate those actions may result in your removal from the premises. You understand that all ST KBPAP events, concerts, and programs are to be considered a family friendly venue unless otherwise noted. You understand that all language, dress, and behavior on and off stage during a ST KBPAP event, shall be appropriate for a family venue, without use of vulgar, derogatory, obscene, or sexually explicit language or behavior. You agree to care for ST KBPAP equipment and property that you use. You also agree to assume responsibility for either injury and/or property damage which result from your actions. Student(s)/ Performer(s)/Entertainers(s) hold ST KBPAP harmless from such injury or property damage. You acknowledge that you have read and understand the policies and agree to adhere to the standards listed above and acknowledge that you have read and understand the student handbook fully and will abide by its policies and procedures. You also commit to attending the classes, lessons, or event that you have registered for, for full duration, unless otherwise discussed and rescheduled with our Office Administrator.By signing below you acknowledge that you have read and understand the policies and agree to adhere to the standards listed above. *
