I certify that I have made true, correct, and complete answers and statements on this volunteer application in the knowledge that they will be relied upon in determining my fitness to be a volunteer. I acknowledge and accept the risks and liabilities involved with my participation in Learning Edge Tutoring. I acknowledge that Learning Edge bears no responsibility or liability for volunteer-student contact or activities occurring outside of tutoring sessions at Learning Edge sites and that any such outside contact or activities are not part of the tutoring program provided and administered by Learning Edge. I understand that volunteers are not to transport students, and I acknowledge that Learning Edge bears no responsibility or liability for any loss, damage or injury resulting from the unauthorized transportation of students. I understand that Learning Edge may wish to photograph or film activities related to its tutoring program for promotional purposes and I authorize the use of my likeness for this purpose.  If I do not wish to be photographed or filmed, it is my responsibility to notify the photographer or filmmaker and Learning Edge at the time such photography or filming is occurring.

I acknowledge the contagious nature of the COVID-19 virus and acknowledge that Learning Edge Tutoring adheres to the CDC recommendations to help prevents its spread. I further acknowledge that no guarantee exists regarding whether or not I may contract COVID-19. I understand that the risk of becoming exposed to and/or infected by the COVID-19 virus may result from the actions, omissions, or negligence of myself and others, including but not limited to, paid staff and others. I acknowledge that I may increase my risk of exposure to COVID-19 by participating in tutoring activities. In the event I test positive for COVID-19 within Forty-Eight (48) hours of the activity, I hereby waive confidentiality and authorize Learning Edge Tutoring to disclose this information to other program participants.  I agree that I will not attend an in-person tutoring session if I am experiencing any symptom of illness such as cough, shortness of breath, difficulty breathing, fever, chills, muscle pain, headache, sore throat, or new loss of taste or smell.  I hereby release and agree to hold Learning Edge Tutoring, its Board Members, employees, and volunteers harmless from any causes of action, claims, demands, damages, costs, expenses, and compensation for damage to myself that may be caused by any act, or failure to act, or that may otherwise arise in any way while I am participating in Learning Edge Tutoring activities. I fully understand that this release discharges the aforementioned from any liability with respect to bodily injury, illness, death, medical treatment, or in connection to my participation as described above.
