To apply for a retest, this form must be filled out completely and submitted electronically.  You have three calendar days from when your original test was physically returned to make corrections then apply for retest. Retests must be approved by Mr. Hansen.

Here are the conditions:
1.)  Students are allowed a maximum of two (2) re-Tests per Semester. Choose wisely.

2.)  All homework/classwork assignments have to be turned in for the Chapter/Unit to even qualify.

3.)  Prior to re-test, on a separate sheet of paper, you need to correct ALL problems you got incorrect and explain what you did wrong.

4.)  Re-Test items will be at teacher's discretion and will NOT be the same questions originally assessed.

5.) All re-Test opportunities must be performed during early-lunch or after school; never during class.

6.) Must retain possession of original Test and present it upon re-testing appointment.

SCORING TIERS

- Students originally scoring <70% will have an opportunity to earn a new score of up to 80% dependent upon performance.

- Students originally scoring between 70% & 80% will have an opportunity to add up to 10% to their original score; dependent upon performance.
 
- Students originally scoring > 80% will have an opportunity to add up to half of their missed % back. Ex.) Could jump from 82% to 91% or 90% to 95%.
