PARENTAL CONSENT FOR AN EDUCATIONAL VISIT

School Group: CHALFONT ST GILES JUNIOR SCHOOL – YEAR 4

Visit Summary: BAPS Shri Swaminarayan Mandir (Neasden Hindu Temple), Pramukh Swami Road, Neasden, London, NW10 8HW    

Date and Time: Monday 22nd April 2024, 9am to 1:30pm

Dress Code – school uniform to be worn. Please note that a strict dress code operates within the temple complex. Specifically, tops must cover the shoulders, chest, navel, and upper arms. Leg-wear must be at least below knee-length. 

Shoes – note that it is customary to remove footwear upon entering any part of the Haveli complex. Shoe racks are provided within the complex. 

Children will need to bring a packed lunch to school (this will be eaten when we return to school so this can be in normal lunch boxes/bags).

By completing this form I am giving consent for my child to go on the Year 4 visit to the Neasden Hindu Temple on Monday 22nd April. I have read the relevant information letter.

I agree to his / her participation in the activities described and I acknowledge the need for and expect him / her to behave responsibly. I have completed a trips consent form when my child started at the Junior School and I can confirm that I have updated the school with any changes to contact details or medical conditions and additional medical information may be given below. I agree to complete and return this consent form by Monday 25th March 2024. 

I understand that there is a voluntary contribution of £10.90 requested for this visit. This is a voluntary contribution to cover the costs of the coach, but if we do not receive sufficient funds to cover costs then the trip will not be able to go ahead.
