The Maryland Primary Care Program (MDPCP) wishes to assess the current COVID-19-related equipment needs. The responses and information received from each individual practice will allow us to assess need by site, then work on your behalf within the Maryland Department of Health to advocate for the provision of key equipment to your practices.  We cannot help you if we do not know what you need.

Please complete the survey below to help the Maryland State Government discover and understand your COVID-19-related equipment requirements.  Again, the MDPCP PMO asks each individual MDPCP practice to complete this survey by the close of business hours on Wednesday, May 20, 2020.   Please note that unrealistic estimates of need will not be acknowledged.
