HEALTHY LIFESTYLE HONOR CORD APPLICATION
The following application petitions the department for consideration of receiving an Honor Cord at the time of Graduation. Submit only completed applications before deadline. The Department will submit approved forms to the Counselor’s Office. You may apply to more than one department, however, an application is required for EACH INDIVIDUAL HONOR CORD.
