Education Program Request Form
Thank you for you interest in booking an education program with the Museum of Health Care at Kingston! For more details about each program, please review the Education Programs Guide on our website.

Please note, your booking is not confirmed until you have received a response from us. Completion of this form does not guarantee your preferred date or time. We will do our best to fulfil your request!

Programming is available Monday-Friday between 9:30am-4pm. Should you required alternate days or times, please indicate so on the form.

Program Location
On-site and virtual programming is held at the Museum of Health Care (Ann Baillie Building, NHS) 32 George Street, Kingston. Select off-site programming (we bring the Museum to you!) is also available.

Fees 
Education programs, guided tours and adult/senior program cost are $5 per participant. 
Staff/volunteer/drivers are free. Cost should never be a barrier to accessing programs at the Museum. Please contact us should you need fee assistance.

Program Running Times
With some exceptions, elementary school and high-school on-site programs run best when given 2 hrs. Should you wish a program to be shorter, please indicate so on this form. 

If you wish programming to run beyond 2 hours, extra fees will apply. Costs are an additional $1 per student for any requested extra time or activities.

All day programming is available upon request! (extra costs apply)

Capacity Restrictions
Given the nature of our space, groups larger than 35 will need to be separated. Please contact us for large group solutions!

Accessibility
The Museum of Health Care main floor is accessible by elevator. Much of the Museum is accessible to those with mobility needs, however our public washrooms are not fully accessible. There is no on-site parking for the Museum and the parking lot in front of the Museum is unpaved. Please contact us if you should have any accessibility concerns.
