Thank you for your interest in the Alliance for Health Equity. The Alliance for Health Equity is a group of over 30 hospitals, local health departments, and community organizations in Chicago and Suburban Cook County that are working together to conduct a Community Health Needs Assessment (CHNA). The information collected during the CHNA will help Alliance partners develop or expand policies and programs to address priority health needs in the communities that they serve.

Please complete this form if you would like to receive updates about the results of the overall CHNA, the community input survey, and focus groups. Once completed, all CHNA data reports will be available for review and use by communities.
