This form must be completed once per school year for each family of participants in Hope Lutheran Church programs. This form is valid through August 2024. Any changes to the information on file should be relayed to Grace, or the church office promptly. Thank you!
PLEASE READ: Fill out one form per child/youth. You only need to fill out the parent and emergency contact section on ONE of the forms submitted if you have more than one child.
Our programs look very similar to last year! JAM, Confirmation, Youth After School Hours, High School Lunch Bunch, the Mentorship Program, Soup + Song, and Family Fun Opportunities will continue much the same. Detailed information for each of our offerings can be found here.
Registration Payment of $30 per child can be made through subsplash, or the church office. We don't ever want money to get in the way of participation, so no-questions-asked scholarships are also available.
