LAS LOMITAS ELEMENTARY SCHOOL DISTRICT SHORT TERM INDEPENDENT STUDY MASTER AGREEMENT
In accordance with the LLESD School Board Policy on Short Term Independent Study, parents requesting a short-term independent study plan (SISP) must sign a Short Term Independent Study Master Agreement.  Requests are to be submitted 2 weeks prior to the start of the absence:

Objectives of Short Term Independent Study:

1-The general objective for the term of this agreement is to complete the coursework necessary to keep the student current with the assignments missed during the absences from class of three days and less than 15.

2-According to district policy for independent study in K-8, no more than 5 days may elapse between the date the assignment is made by the teacher and the date it is due.

3-The Las Lomitas Elementary School District will provide the teacher services, instructional materials, and other necessary items and resources as specified for each assignment.

4-Method of study: Specific methods of study will be designated on the “Student Assignment and Work Record”

5-Form and are incorporated herein. Examples of methods of study for the student will include: Independent Reading, Textbook Activities, Problem Solving, Study Projects, and Drill and Practice.

6-Method of Evaluation: Academic evaluations will be designated on the Assignment and Work Record and are incorporated herein. Other acceptable methods of evaluation include, but are not limited to: Student Conferences, Progress/Report Cards, Work Samples and Observations.

The student and parent(s) understand the following:

1-Short Term Independent Study is a form of education that you have voluntarily chosen.

2-If student has an Individualized Education Program (IEP), the IEP must specifically provide for his/her enrollment in Independent Study.

3-The student has the same rights as other students and is entitled to textbooks, supplies, and all the resources available to other children enrolled in this grade.

4-If the student does not complete the assignments and turn them in at Due Date, the absences will be considered unexcused and no credit will be given for independent study.

5-Learning objectives are consistent with and evaluated in the same manner that they would be if student was enrolled in a traditional school program.

6-The parent is responsible for the supervision of the child while he/she is participating in Independent Study. The parent will provide instruction and assistance and an appropriate environment for study.

7-The parent is responsible for ensuring that the work is completed and submitted at Due Date for evaluation. The work must include the student’s name, teacher’s name, date, and subject.

8-SUBMIT ASSIGNMENTS TO CLASSROOM TEACHER. (Assignments not completed will result in unexcused absences.)

9-The student will complete the assignments and return the work along with the Assignment and Work Record Form for the teacher to evaluate at Due Date.

10-Credit given for assignments will result in one day of attendance for each day completed and evaluated by teacher (see Assignment Form sent by the classroom teacher).
