2020-2021 Student Leader of the Year (SLOY) Nomination Form
Phi Lambda Sigma is now accepting applications for the Student Leader of the Year (SLOY) award. SLOY is an annual recognition program sponsored by Phi Lambda Sigma and the Office of Student Affairs. This award recognizes a student who has gone above and beyond to serve as a leader at UIC College of Pharmacy and demonstrates the utmost commitment to the profession of pharmacy.

Recipients of the Student Leader of the Month Award during the 2019 Fall Semester are automatically nominated for the SLOY award. All other nominations must be submitted by Friday, February 5th, 2021 at 11:59pm. Nominated students who wish to be considered for the SLOY award must submit a formal application by Friday, February 26th, 2021 at 11:59 PM. The application requirements and submission portal will be released on Monday, February 1st, 2020 after 6 PM.

The PLS Award Committee will evaluate all submitted applications and determine the recipient of the 2020-2021 Student Leader of the Year. The SLOY recipient will be recognized at this year’s UIC COP Honors Convocation.
