2023-2024 Madonna Faith Formation Registration

Our mission in faith formation is to collaborate with parents to form a new generation of Catholics who know and love Jesus Christ and live as his disciples in the community of the Catholic Church. Toward that end, we strive to offer a unified, solid and engaging faith formation program.

Our sacramental programs are both 2-year programs. First Communion preparation classes are offered starting 1st grade, and confirmation preparation classes are offered starting 8th grade.

Madonna Faith Formation Program is made possible through the dedication of volunteer teachers who prepare the students for the Sacraments, and provide religious instruction on how to live as Jesus' disciples.

Classes begin on the third Sunday in September and finish in May.

9:15AM- 10:15AM PreK-4 to 7th grade 

10:30AM-11:30AM Family Mass

11:30AM-12:30PM- Confirmation (8th & up) and Youth Group

Registration Fee: 

After August 1st: $125 for 1 child, $200 for 2 children, $275 for 3 children & more 

Scholarships are available for families in need

General Permission

I request that my child(ren) listed above be allowed to attend Madonna Faith Formation Program located at Church of the Madonna in Fort Lee, NJ for the duration of the 2023-2024 school year. I hereby release and agree to indemnify and hold harmless the parish, its staff and their employees and agents, volunteers, and the Archdiocese of Newark from any and all liability, for injuries, damages, medical expenses, or any other loss to my child or family, including attorney fees, arising from claims of any kind or nature whatsoever from my child’s participation in this program. 

Medical Permission Form

I grant permission for the administration of First Aid to amy child(ren) listed above by the people in charge of Madonna Faith Formation Program at Church of the Madonna, to sign the necessary releases as may be required, and to make the necessary referrals to qualified physicians for the treatment of illness or accidents of a more serious nature. I understand I will be promptly notified in the event of any serious illness or accident and prior to any major surgery, except when delay in such communication would endanger life. In the case of a medical emergency, I understand that every effort will be made to contact the parent/guardian of the participant. In the event that I cannot be reached, I hereby give permission to the physicians selected by the adult staff to hospitalize, secure proper treatment deemed necessary for my child(ren). 

Videotaping and Still Photographs

Video, still photographs, and audio recordings may be taken during the Madonna Faith Formation classes and events. This authorization form constitutes permission for my child(ren)’s participation in videotaping, still photographs, and/or audio recordings, which may be used for future promotional purposes including but not limited to websites, social media, bulletin publications, and bulletin boards.
