I understand that the attendance at Middlesex Agility Club and at competitions and other club events is not without risk to me, my dog, or any dog under my control, and members of my family and my guests who may attend, and property. Some of the dogs to which I/we will be exposed may be difficult to control and maybe the cause of damage or injury, even when handled with the greatest care. I further understand and agree that my attendance at Middlesex Agility Club for training sessions, other events and competitions and that the use of any of the agility equipment, facilities or premises is at my own risk and I freely assume all such risk.

I hereby release and discharge the Middlesex Agility Club, its executive and members, instructors and agents, and the owners of the facilities and premises at which any Club activities may be held from all liability of any nature, for injury or damage which I or my dog or members of my family or my guests or our property may suffer, including specifically, but without limitation, any injury or damage resulting from the action or inaction of any dog or its owner or handler or by the Middlesex Agility Club, its executive, members, instructors and agents, or the owners of the facilities and premises, whether through negligence or otherwise. I expressly assume the risk of all such damage or injury while attending any training session or competition or other event of the Middlesex Agility Club, or while in or on the training facilities, premises, grounds or the surrounding are thereto and waive any and all claims relating thereto.

I hereby agree to indemnify and hold harmless the Middlesex Agility Club, its executive, members, instructors and agents and the owners of the facilities and premises at which any Club activities may be held of and from any and all claims, or claims by any member of my family or any other person accompanying me or attending as my guest at any training session or competition or other event of the Middlesex Agility Club or while in or on the training facilities, premises, grounds or the surrounding area thereto, including any claim arising as a result of any action or inaction by any dog, including my own, or its owner or handler, or by the Middlesex Agility Club, its executive, members, instructors and agents or the owners of the facilities and premises, whether through negligence or otherwise.

I understand that the Middlesex Agility Club, its executive and instructors reserve the right to require the handler of any dog at any time to remove the dog from training sessions, competitions or the training facilities, premises or grounds for any reason they deem appropriate, and I agree that upon being asked to do so, I will immediately remove my dog as requested.

I acknowledge that I have been told to read this form before signing it, that I have been given the opportunity to do so and have in fact done so, that I fully understand the contents of this form and that I am signing this form voluntarily and of my own free will.
I verify that my dog(s) is (are) in good health and has (have) now and will continue to have up to date vaccinations.
