Pathways Community HUB Institute® Model Care Coordination Training

The PCHI Model Training is a requirement for everyone affiliated with a Pathways Community HUB. This 20-hour virtual class available through the Blackboard-PCHI learning platform, focuses on the PCHI Model, including the 21 Standard Pathways, data collection forms, and Learning Modules. 

PCHI is offering sessions of the Pathways Community HUB Institute® Model Care Coordination Training at no cost for up to 2 staff members per PCH/PA.  For more than 2 staff members, the tuition is $500 per person. 

As part of the Pathways Community HUB Institute® Model Care Coordination Training/Trainer Program, following the completion of the 20-hour PCHI Model Training, participants can elect to complete the 4-part evaluation portion of the training including a review exam, case study, teach back, and initial PCH/PA training observation.  Participants who successfully complete the 4-part evaluation can receive a certificate authorizing them to train others affiliated with their Pathways Community HUB or Pathways Agency on the PCHI Model for one year.  Certificates are renewed annually with a refresher training. This is at no cost for up to staff members per PCH/PA. For more than 2 staff members, the tuition is $750 per person.
