School Closure Communication Consent Form
Bigs, you are getting this because yours is the contact information I have on file, but I am asking that you take it to a parent or guardian for the consent, so they are aware of what we are trying to do.

Dear HS Big Parent and Guardian,

Thank you for allowing your child to serve as a Big in our school-based mentoring program. Given the current school closures, we understand that this can be a difficult time for your families to try and navigate. While our school-based program's rules and guidelines do not allow for Bigs and Littles to exchange contact information (phone, email, address, etc.), we are able to make exceptions in order for our matches to stay connected in these difficult and uncertain times so our kids can have just a little bit of normalcy and someone else to talk to right now.

By completing this form, you are giving your high school-aged child permission to exchange contact information with their elementary-aged Little to stay connected virtually, by phone, or mail during this time. This connection will look different between matches, and we encourage you or your child to connect with their Little's parent/guardian to decide what the most appropriate kind of communication will be for their match. Most contact will have to be through the Little's parent or guardian as most of the kids do not have phones or technology that would allow them to communicate with their Bigs without parental help.

Once the parent/guardian of both the Big and the Little have completed this form, the program coordinator working with your child's school will take charge of initiating the communication between match parties. If you have any questions or concerns, please contact the program coordinator who works with your high school student. Make sure to let them know your student's name in any written communication.

*This release is effective from the day that it is completed, through the day that in-person schooling resumes in the 2019-2020 school year*
