All application materials (Application Form, Entry Fee, Member Dues, Biography and/or Artist Statement) must be submitted no later than midnight, December 15, 2020 in order to meet deadlines.  

Please note: Membership in ArtForms is required from the organization or exhibiting artist. If your membership has lapsed or is due before March 1, 2021 you must renew your membership when you send in this application form.  While not required, artists are encouraged to submit a short biography and/or an artist statement at the time of application.  Entry fee and dues may be combined in one payment.
