MEAL ORDER FORM (BELLBROOK ONLINE ACADEMY,STUDENTS LIVING IN COMMUNITY, AND QUARANTINED)
USDA  announced that free meals are back!  Children ages 1-18 living in the Bellbrook-Sugarcreek community are eligible to receive free meals through the end of school year 2020-2021 with no charge to student accounts on Payschools.  Effective immediately, we will provide free breakfast and lunches for pre-order for students not attending in-person learning as well as students quarantined.  You will need to fill out this form. We recommend that you choose "standing meals" and have someone pick up your meals.  You do not need to have children present, however you will be issued a "Meal Pick Up Card" that will need to be presented only once for every pick up day.  If you have someone picking up meals for your family, that card will then need to be presented to our food service staff.

Meals will be available every Monday and Thursday at Bellbrook High School at the triple doors facing the soccer field (Door E) from 2pm-245pm for each child signed up.   Please park your car or pull to the side as to not block any parked cars.  You can then walk up to the triple doors where food service staff will be there to assist you.  Please bring a mask and ensure you are 6 feet away from the next family. Each family will be issued "Meal Pick Up Cards" to be used each time you pick up meals.

On Monday, you will receive breakfast and lunch for Tuesday, Wednesday, and Thursday.  On Thursdays, you will receive breakfast and lunch for Friday and Monday. Please keep in mind that you will always receive your meals for the days going forward, not the day that you pick up. For example, if you pick up on Thursday, you will receive Friday and Monday.  You would have received Thursdays meal on Monday.

Each bag will include breakfast and lunch that will be cold/frozen at pick up and can be reheated using the enclosed reheating instructions.  Our meals are made to be easily microwaved and simple for students to have a nutritious meal.  Each daily meal will have an entree, fruit, vegetable, and milk.  The menu will follow Bellbrook High School menu and will be the featured adventure meal offered for that day.  Please see link for menus.  https://bellbrooksugarcreeksd.sodexomyway.com/landing/high.  We reserve the right to change menu based on availability.  Any student with a dietary accommodation will need to provide medical documentation to support the accommodation.  You may email Jennifer.hoehn@bss.k12.oh.us for more information.

*THE DEADLINE TO SUBMIT YOUR FORM EACH PICK UP IS FRIDAY FOR MONDAY PICKUP AND TUESDAY FOR THURSDAY PICK UP.  ALSO, IF YOU CHILD IS IN PERSON LEARNING, PLEASE DO NOT FILL OUT THIS FORM AS THEY WILL RECEIVE THEIR MEALS WHILE THEY ARE AT SCHOOL.

***Quarantine Meals
If your child is quarantined due to exposure to Covid-19, you may use the attached form for the assigned quarantine period with start and end dates below.  Once your child is finished with that period of time, we will take them off our list.  If for any reason, the quarantine will be longer than the period initially stated, you will need to go back in and sign up again for the time period mandated for your child to quarantine.  If you have an active case of Covid-19 in your family, we ask that you have someone outside your family pick up meals for your child. This can be a neighbor, friend, etc.  If for any reason you don't have someone that can pick up for you, you will need to contact Jennifer Hoehn at jennifer.hoehn@bss.k12.oh.us to discuss.  We ask that you fill out the form at least 24 hours prior to the Monday and Friday pick ups.  If you are past the sign up cut-off, then you will need to do the next schedule pick up.  You may contact Jennifer Hoehn at 937-848-5001 ext. 6105 to see if there is a possibility of the kitchen having the ability to accommodate a short notice pick up.
