2024-2025 NEW STUDENT Pre-Registration Form
We would like to invite students NEW TO MADISON to pre-register for the upcoming school year.  Please complete a form for each NEW student registering.

NEXT STEPS:

For NEW students entering Preschool, please complete this form.  You will also need complete a paper packet that you can download from here.  Once complete, please return the packet to your home school.  

For NEW students entering Kindergarten, **STOP** please complete this Kindergarten Pre-Registration Form - DO NOT complete this NEW STUDENT form.  The form linked above is specifically for Kindergarten students and contains specific information on registration.

If you are a NEW student who has NEVER attended Madison, but wishes to enroll, please complete this form and will contact you in early July after our rollover to complete the New Student Enrollment and Registration.  

Should you have questions over the summer, please contact your building clerk.  If they are not available, please leave a message and someone will return your call.  All building principals will return to their buildings by the third week of July and will be available for questions.  We will contact you in early July when the online enrollment/registration opens for the upcoming school year so that you can complete the registration process.

Each school will be planning registration days over the summer and Open House events before school starts.  While buildings are closed for the summer after summer school wraps up mid-June, our staff will be back in the buildings the third week of July.  We will communicate this information as soon as it is available.
