CW 2022-2023 Residency Form
Students may not enroll until residency is verified with the exception of homeless students.
All new students and students in families that have moved out of the District during the past school year must register their residence with the Unit Office. Families or legal guardians may be required to show proof of residency by two or more of the documents listed below:
Proof of home ownership, drivers license, rent receipt, utility receipt, voters registration, or any other document that shows a legal address in the Casey-Westfield District.
Students may attend Casey-Westfield CUSD #C-4 Schools on a tuition-free basis if they meet one of the following requirements:
1. The individual with whom they are residing lives in the school district and has assumed custody of the student as evidenced by form 7:60-E3 Evidence of Non-Parent’s Custody, Control and Responsibility of a Student.
a. Residency cannot be established for the sole purpose of going to school.
b. Legal guardians must have the sole care, custody, control and support of the
student.
2. The parents or parent with whom they are residing lives in the District or is an employee
of the District.
3. The student is of legal age and is emancipated and has an established residence of their
own, provided they are self-supporting and are not under the care, custody, control or
support of their non-resident parents or guardians.
4. The student is considered homeless because the student’s current living condition meets
one of the following situations:
lives in a shelter, lives with relatives or others due to lack of house, lives at a train or
bus station, park, or lives in a car, lives in a motel/hotel, camping ground, or other
similar situation due to the lack of alternative, adequate housing, lives in an abandoned
apartment/building, temporarily housed in a shelter, awaiting
DCFS permanent foster care placement, or any other similar situation.

Students who do not meet the above requirements are not entitled to attend Casey-Westfield CUSC #C-4 on a tuition free basis. Students wishing to attend school on a tuition basis must have permission of the Board of Education of the Casey-Westfield Community Unit C-4.
