Payment - I agree to pay the Alamo City Track Club monthly fees of $100 on a month-to-month basis. I understand payments are due the 1st of the month and there are generally no refunds. I understand and agree that my child will be registered with the AAU and will be covered by their comprehensive insurance program while participating in team activities. *
Waiver - I give permission for my child to participate in the Alamo City Track Club and all activities involved with the program. I wave and release any and all rights and claims for damages which I or my heirs may have against the Alamo City Track Club, any club sponsors, and any club agents, representatives or assignees, for any and all injuries, loss or damages which may occur while my child is a participant in the program. I realize that there is an inherent risk in these activities and I am willing to accept the results of those risks. *
Photo / Video Release - I understand that as a participant, myself or my child may be photographed and/or videotaped during participation in the Alamo City Track Club. These photos/videos may be used in social media, presentations, and/or promotional materials. I release the Alamo City Track Club to use these photos and/or videos. *
Emergency Medical Care - I consent to emergency medical care for my child in case of emergency, sickness, or injury where the parent or legal guardian is not immediately available.

Permission to participate - I acknowledge that I have freely and voluntarily entered into this agreement and that I have read and understand all the questions in this registration process in their entirety. I hereby give my consent for the above Athlete/child to participate in the Alamo City Track Club Program. *
CODE OF CONDUCT: I pledge to be responsible for my words and actions while attending, coaching, officiating or participating in a youth sports event and shall conform my behavior to the following code of conduct:

 

1. I will be a positive role model for my child and encourage sportsmanship by showing respect and courtesy, and by demonstrating positive support for all athletes, coaches, officials and spectators at every meet, practice, or other sporting event.

2. I will not engage in, nor encourage my child or any other person to engage in, any kind of unsportsmanlike conduct with any official, coach, athlete, or parent such as using profane language or gestures, etc.

3. I will not engage in nor encourage any behavior which would endanger the health, safety or well-being of any coach, parent, athlete, participant, official or any other attendee.

4. I will not encourage my child, or any other person, to engage in any behavior which would endanger the health, safety or well-being of any coach, parent, athlete, participant, official or any other attendee.

5. I will teach my child to play by the spirit of the rules and to resolve conflicts without resorting to hostility or violence.

6. I will treat coaches, parents, athletes, officials or any other attendees with respect. I will encourage my child to do the same.

7. I will not engage in, nor will I encourage my child to engage in, verbal or physical threats or abuse aimed at any coach, parent, athlete, participant, official or any other attendee.

8. I will respect the officials and their authority during games and will never question, discuss, or confront coaches or referees at the game field, and will take time to speak with coaches at an agreed upon time and place.

9. I will refrain from coaching my child or other players during games and practices, unless I am one of the official coaches of the team.

10. I will inform the coach of any physical disability or ailment that may affect the safety of my child or the safety of others.

I hereby agree that if I fail to conform my conduct to the foregoing while attending, coaching, officiating or participating in a youth sports event I will be subject to, including but not limited to, the following in any order or combination:

1. Verbal warning by Club Coach.

2. Written warning issued by a Club President.

3. Suspension or immediate ejection from a youth sports event by any individual with authority to do so.

4. Season suspension, multiple season suspension, or ban issued by Club President.
