If you have a different email you would like included in the directory, please enter it in the appropriate section in the form. If you are filling the form out for another member of your family, please include their email address as it should appear in the directory in the appropriate section in the form.
This form is to update your information or the information of a family member for our Church database. This information is utilized for internal purposes and for publishing in our Member & Friend Directories unless you opt-out* of publication. If you opt-out, your information is used for office-use only.
If you would like to submit a picture for the directory, please email aa@uufresno.org. PLEASE NOTE: Family pictures including children (under 18) are welcome, but it is up to parents' discretion and requires permission to print below. However, for the safety and well-being of our youth, only children(s) names are published, not individual pictures.
