Zoo Field Trip - Chaperone Interest Form
Please complete the following questions if you are available and interested in serving as a parent chaperone for our zoo field trip on Thursday, April 18th. More information will be given via email as we get closer to the date.

Parent Chaperone Responsibilities include:
-be available to remain with a group at the zoo from 9:00-2:00pm
-transportation to and from the zoo (carpooling is okay if you coordinate it with others)
-be responsible for a small group of students while walking throughout the zoo 
-carry student sack lunches throughout the day in a bag/backpack
-contact teacher with any questions/concerns throughout the day
-bring students to the meeting point at a requested time
