Hello TDs,
Please use this form to submit a report of your recent event(s). As a reminder, this report is due 72 hours after the conclusion of games at your event. Until this is completed, you cannot TD a new event.To complete this form, you will need the following information and items:
- Name and location of event
- Name(s) and certification(s) of all medical staff working at your event
- List of injuries (injury type, whether or not the individual sought additional treatment beyond the on-site medical staff such as at an ER or urgent care)
- Straight red cards: player name, player's team, opposing team, head referee of game, and card type
- Any other conduct violations you need to report (unsportsmanlike conduct outside of a game, violations of the player code of conduct, etc.)
- Photos of scorecards for all official games
