Please provide the information below about your child.

The school will require copies of the following documents when you first visit the school (or scans at the time of completing this application form):

       i) One passport style photograph.
      ii) The student’s three most recent school reports.
     iii) The student’s birth certificate (Thais and Non-Thais).
     iv) A copy of the student’s and parent’s passports (Non-Thais) or national ID cards (Thais).
      v) A copy of the family’s house registration (Thais only).
