Appointment Information:
-Please make sure to fill out this form and the attached document AT LEAST 24 hours before your appointment so that we can better assist you.
-You can upload the document directly to your MyChart or bring a printed copy to your visit.
-Please arrive 30 minutes prior to your appointment
-Please bring a hungry baby to your appointment!
-Please bring all breastfeeding supplies to your appointment (shields, flanges, etc.) that you currently use to feed your baby.
-If you need to cancel or reschedule, please kindly provide a 24 hour notice prior to - 248-625-9755
