WELCOME!

This online form will collect information to register your child to participate in VBS.  It must be completed by a parent or legal guardian.

Kindly fill out one form per child from Preschool through 5th grade.  If your child will complete 6th grade prior to VBS then your child should fill out the Apprentice Job Application form.

We first will ask for your contact information, including your email address, as well as the name and age of the child that you are registering.  We will also ask about allergies and medical conditions, and for your permission to use photographs and videos of your child.

We will try to accommodate requests.

Preschool Crews to not rotate between activities.  If you request that a Preschool child be placed with an older child, the older child will be put into the Preschool group.

Photo-video release permission *
