Failure to have a completed roster in the GotSport registration system may lead to the team's disqualification from the event. Any extenuating roster circumstances must be approved by the Tournament Director in writing before the event begins. I understand that players are only allowed to participate on one team, regardless of age group, during the event. *

I understand that I need to carry a valid medical release form for all participating players playing in this event, and further, I assume liability for the consequences of not carrying player medical releases. Teams may use the USYS or US Club Soccer Medical Release form which is required for registration. *

I understand that referees and tournament directors have the right to ask for age verification, or affiliate registration at any time during the event. If age or registration verification cannot be provided within one hour of the request, then the player or team in question may be disqualified. *
