Lateral Flow Testing Consent – September 2021
 
As per government guidance issued to schools, all students are encouraged to undertake two on-site lateral flow tests upon their return in September 2021. These tests will be administered during the first two week in the new term. All students require two tests 3-5 days apart. In order for students to be able to be tested we require parents/guardians to provide consent to this. Please ensure you read the privacy notice (http://ow.ly/Nf9050FQuTL) and fill in the form below to provide your consent to continue asymptomatic testing and help keep our academy community safe.

This consent covers the initial mass testing of students in early January 2021 and any subsequent close contact testing required for positive cases that occur in the academy in the future. *
