A FORM NEEDS TO BE COMPLETED FOR EACH INDIVIDUAL ATHLETE even if they are siblings.

If you are unable to make one of the dates please indicate why in the Special Notes Section. Alternate Tryouts are not guaranteed if you can't make one of the original dates.

15u is Grad Year 2027
16U is Grad Year 2026
17u is Grad Year 2025

If there is a reason you are choosing not to tryout for your Graduation Year please indicate it in the Special Notes Section.
