QUFSD Meal Delivery
The USDA has extended the program of providing free breakfast and lunch for children 18 and under in the district. DURING SUMMER 2021, if you would like to have meals delivered for you children, please fill out this form.

Please note, meals will be provided for summer academy students. Do not complete this form if your student is attending summer academy.

Deliveries will be Monday, Tuesday or Wednesday with meals for a week.

Deliveries will start the week of July 5 and tentatively go through Aug. 18.
