DEADLINE TO COMPLETE FORM: May 5, 2023 by 12 PM

This form is to be completed by a resident whose license is ending at the end of the academic year (May 12, 2023 at 2pm) and would like to request additional time to move out due to unavoidable circumstances. Extensions will be approved to stay until Monday, May 15th at 12 noon. Residents will be charged a nightly rate of $40 for each night they stay, beginning with late check-outs for any period after Friday, May 12th at 2PM. An additional nightly stay is defined as each additional date that the unit is still occupied past 5pm. The form must be submitted by the deadline for consideration for a license extension.

Residents who have received a summer assignment do NOT need to submit this request. Information regarding summer transition will be sent with summer assignment information.

You will receive a response to your form regarding your status no later than 5pm on Friday, May 6th.
