D88 School Meals Survey
We are getting ready to start the school year and are asking parents to fill out this quick survey to get an idea about families' plans for students' meals for the 2020-21 school year.  This is not a commitment for your family to abide by your answers; we are just trying to be prepared for the number of meals we may be serving when school starts.  Thank you for your assistance and best wishes for a healthy and safe school year!

The learning option for my students for the 2020-21 school year is as follows:

ALL STUDENTS ANSWER QUESTIONS 1 & 2 BELOW:
1.  If, for some reason, the district has to return to remote learning for all students, I would pick up meals for my students 1 or 2 times per week IF IT WAS FREE FOR ALL STUDENTS (similar to the summer meal service that was offered.)
2.  If, for some reason, the district has to return to remote learning for all students, I would pick up meals for my students 1 or 2 times per week, IF MY CURRENT LUNCH STATUS APPLIED? (For example, a) there would be no charge for meals if family is free status, b) breakfast would cost 30 cents and lunches 40 cents, if family is reduced, or c) meals would be charged full price for those that are full pay).

3. My students will be 5 day a week remote learners (home every day of the week).  I would be interested in picking up meals for the week (5 breakfast and 5 lunches) on Mondays each week school is in session for all of my students.
