Parent/Legal Guardian Consent and Waiver of Liability

I understand that the Summer Social Program and related activities offered by Pathways to Employment have inherent risks that may arise from the activities themselves, my own actions or inactions, or the actions or inactions of Pathways, its directors, officers, employees and agents, other volunteers, and others present at Pathways program sites.  I assume full responsibility for any and all risks of bodily injury, death or property damage caused by or arising directly or indirectly from my presence at Pathways program sites or participation in Pathways Summer Social Program activities, regardless of the cause.  Further, in consideration for my participation in this Pathways program, I waive and release any and all claims against: Pathways; the owner(s) of premises on which Pathways Summer Social Program activities take place (collectively, “Landowners”); the entities which fund any Pathways programs I participate in (“Funders”), and Pathways Funders’ and Landowners’ directors, officers, agents, employees, volunteers and affiliates (collectively, the “Released Parties”), for any liability, loss, damages, claims, expenses and attorneys’ fees resulting from death, or injury to my person or property, caused by or arising directly or indirectly from my presence at a Pathways Summer Social Program site or participation in related activities, regardless of the cause and even if caused by negligence, whether passive or active, except and only to the extent any injury is caused by the gross negligence or willful misconduct of the relevant Released Party. I agree not to sue any of the Released Parties on the basis of these waived and released claims.

In addition, if I am a parent/legal guardian of a Participant that is a minor or conserved, by my signature below, I acknowledge and affirm that I assume the above-mentioned risks and enter into the above-stated waiver and release on behalf of the Participant for whom I am the parent/legal guardian.
