This form is for every Patron that hasn't received their Moika merch yet.

I know it's not excuse, but during the months of January to July I was traveling with my family and I had a very difficult time handling some of the cases of missing merch. I apologize for not handling this with the speed it required.

But I still want to make things right for everyone. So, if you are one of the cases where you paid for my $18 tier and you have not received your merch yet, or you received a damaged piece, this form is for you.

- You have until September 24th, 2023, to fill this form. After that I will only be able to do refunds.

- Merch will be sent on the last week of September. Make sure your address is valid at that point and the following weeks.

- I have limited amount of stock, so please don't ask for merch you already got or maybe didn't paid for (I have a payment history for each one. If you have doubts, just select it and I can clarify it later on Patreon Dm's or via Email)

- I will try to send everything with a tracking number, but no promises yet!

- This will be my last time attempting to re-send missing merch. After this, all I will be able to offer are refunds.

I also have an option for those who no longer want it, and may prefer a refund.
