Pets on Parade Dog Admission and Waiver

Dog Rules:
-All dogs must be up-to-date on rabies vaccine
-All dogs must be on a leash at all times -- NO Flex/Retractable Leashes
-Owners must clean up after their dogs
-Pet owners must report any incidents to a FETCH a Cure staff member immediately

1. I will be in control of my dog at all times while at FETCH a Cure's event.
2. I understand that FETCH a Cure is not responsible for any incidents involving my dog(s) at any time and cannot be held liable under any circumstances if I decide to leave my dog(s) in pupcare.
3. All of the information contained in this Dog Admission and Waiver form is accurate.
4. My pet has not been deemed a dangerous or potentially dangerous animal at any time in the past.
5. I agree to the above rules for myself and my dog.

If my pet should bite a person, bite another pet, cause property damage, and/or be directly or indirectly involved in any incident, I agree to the following:

1. Report the incident immediately to a FETCH staff member; and,
2. Keep my pet available until I have received instructions from the staff and/or local authorities regarding the required procedures.
