2023 Transfiguration School Summer Institute
Thank you for your interest in our Summer Institute 2023. Please read and complete the following form to finish your registration process.

Your registration is not complete unless your child has been assessed or is a currently enrolled student. Assessment fee $50. One time fee for new students. Currently enrolled students not charged this fee.

$80 registration fee applied to non-Transfiguration students or currently enrolled Transfiguration students NOT enrolled for the 2023-2024 school year only. 

Once you have completed this registration form and selected your payment choice, please visit www.transfigurationschoolnyc.org/summerinstitute if paying by credit card. 3% fee will be charged separately. Otherwise mail or send in a check with your child written out to Transfiguration School with "Summer" in the memo line. Seat not confirmed until deposit is received. Checks must be received within one week of registration. No cash.

A non-refundable $500 deposit is due upon registration to confirm a seat for your child(ren) in the program. The balance will be due May 15, 2023.

Registration must be completed for each student enrolling for the 2023 Transfiguration School Summer Institute. If you have more than one child participating, you will need to complete this form for each child.

In addition to your registration below, please prepare to submit your child's health examination form. Children must meet the New York State school vaccination requirements for their age.
