Sports Physicals Sign-Up for July 21st
Please sign-up for a block of time for your sports physical.  A new form will need to be filled out for each child.

PRIOR TO THE PHYSICAL, YOU WILL NEED TO PRINT THE FOLLOWING FORMS AND BRING THEM FILLED OUT:  IESA PRE-EXAMINATION PHYSICAL FORM AND CONSENT TO PHYSICAL FORM.  If you cannot print them, you can call the DTHS or DGS office to pick up hard copies.

SOCIAL DISTANCING AND MASKS WILL BE REQUIRED.  PARENTS WILL BE ASKED TO STAY IN CARS.

$25 - Cash or checks made out to DTHS will be accepted.

6th Graders and 9th Graders need a complete physical at a physician's office and will not be accepted on this day.

Please be at the school 10 minutes prior to your appointment.
