Work shadowing involves students attending a place of work with either a parent, other family member or that of another responsible adult with the consent of a parent. We hope that through this opportunity students will gain a true sense of a work-life experience.
This form must be completed by the parent on behalf of the student. The parent/guardian email must be provided as contact may be made for extra consent.
If you wish your child to work shadow for all 3 days and not participate in the CCHS work experience day then please check the box at the end of the form.
